Tulsa Bicycle Club -- Ride Release Form Weekly or Link–Scheduled Ride

The following release must be signed by all riders who are not members of
the Tulsa Bicycle Club and for minors by a parent or designated guardian.
Non-member minors are welcome only if accompanied on the ride on a bicycle by
the signing parent or guardian.

I hereby make known that I know there are risks involved with bicycling in the
State of Oklahoma and agree to hold blameless the Tulsa Bicycle Club, volunteer
ride leaders, workers providing support services and participants in the bicycle
tour scheduled for today, and other dates I might ride, in the event of accident,
injury, death, misadventure regarding myself or loss or damage to my bicycling
equipment. I understand that the routes taken are on the public roads of the
State of Oklahoma on which bicycle riding may be dangerous. I agree to assume
full responsibility if I am injured, killed or suffer loss of property.
